The Endometriosis Health Profile (EHP-30) survey was designed in collaboration with Leeds Beckett University and the University of Oxford to measure the wide range of effects that endometriosis can have on those with disease. Data gathered in this survey will help to provide up-to-date information on the quality of life of those with endometriosis.
The survey should take approximately 15 minutes to complete. Responses are completely anonymous, we are not asking for any personal or contact details that can identify you.
